Pre-Approved Absence Request

This form is used for family days where the student will be absent from school. The Greenways principal will excuse up to 5 of these days per school year, depending on current attendance at the time of submission. This form must be turned in to the campus attendance clerk prior to the absence. A separate form must be completed for each student/campus.
