Request Assistance 

If you find yourself in need of assistance or behavior support for a student, there are several avenues you can access.  

PBIS Referral Form: 

Use this form to request assistance from your PBIS team.  The team will evaluate your concerns, and if possible and appropriate, assign a PBIS case manager to the student in need. If your student is assigned a case manager, we will send you a bi-monthly form to complete to track the student's progress.

If you have a minor concern, or you're not sure what the best route is to address a student's particular need, email us directly, and we can arrange an informal observation of the student.
