When a student has been removed/suspended use the following forms to 1. Determine if a Pattern exists for MDR purposes, and the Suspension Documentation is for building principals and case managers to use to help keep track of Discipline. 

When a student has had 11 or more days of suspension within a given school year, the district must provide FAPE beginning the 11th day and for every removal after during the school year.  Use the forms below to notify parents of what services will be provided and to track services.  If a student does not attend several consecutive sessions, contact your special education supervisor for guidance.
