Secondary transition is the process of preparing students for life after they leave high school, including participation in post-secondary education or training, employment, and independent living. Transition planning begins at age 14, as students explore what they want their post-school outcomes to be through career awareness exploration activities. Transition planning involves a partnership between the student, the family, school-age services, post-school services, and local community members.
Preparing students to pursue their dreams of further education or training.
Preparing all students to be competitively employed or supported through the appropriate continuum of employment services.
Fostering skills for living independently in the community of their choice.
This checklist is designed to support youth with disabilities, as well as their family and IEP team members, to ensure they are reviewing all aspects of transition planning at age appropriate intervals.
