PRE-PARTICIPATION PHYSICAL EXAM
This is required for ALL 9th and 11th grade student-athletes competing in athletics. An athlete's physical exam must be completed and approved on Aktivate by the Athletic Trainer at Boise High School prior to the first day of practice for their sport.
