PRE-PARTICIPATION PHYSICAL EXAM
A physical is required for ALL 9th and 11th grade student-athletes competing in athletics. An athlete's physical exam must be completed and approved on Rank One by an Athletic Trainer at Boise High School prior to the first day of practice to participate.

This is required for all athlete's each year, and is valid for all sports occurring in that school year. PLEASE READ AND FOLLOW ALL INSTRUCTIONS CAREFULLY!

Beginning May 1:
Go to the Rank One login page
Select Add Student
Enter your student’s Date of Birth and Student ID number
Select Start Forms
Upload your student’s physical and required participation documents

Download their student’s current physical- Watch Video
Upload it into Rank One during registration
