Driver's Testing
You MUST have passed 4 classes the preceding semester before applying for a permit.
You MUST NOT have accumulated 9 or more unexcused absences.
After you have received your permit, you must continue to meet the above requirements. If you do not, your name will be turned in to the state department as non-compliant and your permit/license will be suspended.
The permit/license can not be reinstated until a semester has passed.
WHAT MUST I BRING WITH ME TO APPLY FOR A PERMIT OR LICENSE?

Your social security card, which has not been laminated.
An original or certified copy of your birth certificate.
The parent or legal guardian of applicants under the age of 18 must sign the application. (A legal guardian must have a certified copy of guardianship papers.)
No Pass/No Drive Form (also known as school compliance verification form). See your counselor to obtain a copy of the school compliance verification form.
Loss of license or permit can occur for dropping out of school or for academic deficiency in accordance with KRS 159.051/601 KAR 13:070.
