This form is needed by any guest who is a Bethel High Graduate or does NOT attend school. Make sure to print this and have your guest fill it out. You must have it with you at purchase with a printed copy of their driver's license.

BHS Students, underclassmen & BHS Graduates do NOT need this form. 

*All GUESTS must be under 20 years old.
