How to get Started with us:
Must have IRIS (Include,Respect, I Self-Direct) or Must have MyChoiceWisconsin
Must sign an ROI and complete our BLRS Referral Form or call (414-323-5533)
Services that we provide through Family Care are as follows:
Assistance with finding placement for people with minimal, and or complex needs
Assistance with setting up needed services,equipment, supplies,and supports
Transition Services(1st Month's rent, security deposit, furniture, household goods, and moving expenses)
Family Care
Family Care is a Medicaid long-term care program for older adults and adults with disabilities. The goal is to get members the services they need to live in a home setting when possible.
To enroll in Family Care, you must:
Be at least 18 years old.
Be a frail elder or an adult with a disability.
Be eligible for Medicaid.
Be functionally eligible. The Long Term Care Functional Screen helps confirm eligibility.
Have a long-term care condition that will last more than 90 days.
People with dementia and a terminal condition also may qualify.
