Moving Home Minnesota (MHM)
Moving Home Minnesota(MHM) is a federal demonstration project. The goal of this program is to create opportunities for Minnesotans to move from institutions to their own homes in the community; designed to help people find a place to live that’s affordable, sustainable, and safe.
MHM benefits last for up to 6 months.
The eligibility requirements are as follows:
An individual must have resided in a qualified setting or institution for 60 or more days
An Individual must also be eligible for Medical Assistance (MA)

Chooses to receive services
We also offer MHM Comprehensive Community Support Services (CCSS)/Post Discharge Support and MHM Transitional Services
Moving Home Minnesota can also provide continued support to eligible participants after they have moved into the community. Some of those supports may include:
Advocacy on your behalf to potential landlords
Advocacy to help gain access to benefits
Eviction expungement
Household budgeting
Continued connection to community resources
Trasition Services(1st month's rent,security deposit, furniture, household goods and moving expenses
