Covered Services Include, but are not limited to the Following:

• Initial Client Consultation

• Transition Plan Agreement Consultation

• Book movers and coordinate the packing and unpacking of their belongings

• Coordinate third party vendors (up to 3 vendors)

• 2 housing search presentations and 3 housing tours

• Constantly contact to ensure new home/community is ready for client

• Communicating with all parties involved to ensure a smooth transition

• Helping with restoring utilities services and address change

• Coordinate Moving Day

Eligibility

• Must not be eligible for MN Medical assistance

• Must want to transition/move to a new home or environment
