Immunization records turned in schools are required by state law to be medically verified. This means immunization records must be from a health care provider, or you must attach paperwork from a health care provider to your handwritten form that shows your child’s records are accurate. Your child cannot attend school until you provide these records. 

Option 1: Sign up for MyIR to View, Download, and Print Your Family's Immunization Information

Once they receive your signed form, they will register you in MyIR and send you a temporary PIN and instructions on what to do next. When you're done, you'll have immediate access to your family's immunization records to view, download, or print as often as you need. Option 2: Sign up for MyIRmobile.com

Sign up for MyIR Mobile by going to myirmobile.com and following the registration instructions. Your registration information will be used to match your records with the state immunization registry. You will receive a verification code on your phone to finalize the process.

Once registration is complete, you can view your immunization records and access your COVID-19 vaccination certificate. Printed records from MyIR Mobile do not meet school immunization requirements, so please sign up for https://wa.myir.net/ (option 1) for valid school immunization documents.

If you have questions about your COVID-19 vaccination record and want to talk to someone, please call 833-VAX-HELP. Please note operators can only assist with COVID-19 records. If you have other questions about immunization records, please take a look at Option 5 below.

Option 3: Visit Your Local Pharmacy, Clinic, or School

Certain local pharmacies, clinics, and schools can register you in MyIR. If you live near one of these pharmacies, clinics, or schools, visit them today to begin the sign-up process. If you don't see a pharmacy, clinic, or school near you, try one of the other options listed on this page.

Option 4: Request a Complete Immunization Record from Your Healthcare Provider

Most healthcare providers in Washington use the Washington State Immunization Information System. Please ask your provider if they can give you or your child a complete immunization record. They can print it from the Immunization Information System or from their own medical record system.

Option 5: Request a Complete Immunization Record from the Department of Health

If you can't get a complete immunization record from your provider and don't want to sign up for MyIR, contact the Office of Immunization and Child Profile at 360-236-3595 or 1-866-397-0337. The Department of Health uses the Washington State Immunization Information System but does not have complete immunization records for all people. When you call, you'll be asked to provide identifying information, such as name, date of birth, and address. At that time, they will tell you whether or not a record exists in the system, but no other information will be released over the phone. This is to protect the privacy and confidentiality of your information.
