Custody Order and Agreements

The most common kinds of child custody are Legal Custody, Physical Custody, Sole Custody, and Joint Custody.

If applicable, please provide copies to the District Office.

Is the student unaccompanied? Is the family or the student living in a motel, shelter, in someone else's house or apartment with another family, couch surfing, a car, park campsite, or similar location?
