Below is a list of needed forms and documents for the completion of the Standard FBA and BIP at the elementary level. Forms are age based, please only use this section if you are completing a ELEMENTARY FBA/BIP
Elementary Standard FBA and BIP Checklist
This document will guide you through the completion of the Standard FBA BIP process. The checklist consists of two sections. The yellow section lists the documents to be completed by the Teacher of Record. The green section documents are prepared by the Teacher of Record and then completed by the classroom teacher and returned to the Teacher of Record.
Define Target Behaviors Form
The team needs to identify ONE TO THREE target behaviors to focus on. You don't have to have three, but you can have no more than three target behaviors. The more target behaviors identified the more complicated the plan will become. Often less is more. Focus on the things that are most impeding the student's ability to function in the school setting, or to be successful in the classroom. You will need to address any safety concerns as a target behavior. See below for some way to help identify target behaviors if your team is struggling.

This is a list of frequently seen school behaviors defined by a formal operational definition. This is only a list of frequently seen behaviors, it is very possible for your behavior to not be on this list.
Student Interest Inventory - Choose Appropriate Level Below
Forced Choice Interest Inventory
The forced choice interest inventory is appropriate for younger students and students who will struggle to answer when given too many different options to choose from. This inventory is completed one-on-one with the student and then scored. You will receive general information about possible types of preferred reinforcers that maybe preferred by the child.
Student Interest Inventory
This is an interview based interest inventory. It is useful for older students who are able to verbally express different wants as well as independently make a choice among several different options.
Reinforcement Assessment for Individuals with Severe Disabilities (RAISD)
This interest inventory is a structured interview completed with individuals who know the student well, typically a parent. This tool is used with lower functioning students and those who are not able to actively participate in the interest assessment themselves.
Elementary Student Interview
This form is designed to collect information on the students perspective of their behavior as well as some of the students needs and desires with in the school setting. It is designed to be completed with the student together with a staff member. This is a digital form and once downloaded can be typed directly into.
ABC Data Collection - Choose Your Preferred Style Below
ABC data collection periods will vary depending on the frequency of the behavior targeted. ABC data collection is for the purpose of looking for behavioral patterns. Data collection should be taken over 10 school days.
Questions About Behavioral Functioning (QABF) - One For Each Target Behavior
The QABF is a problem behavior checklist used to help identify a behavioral function. List your target behavior in the section labeled "Target behavior being addressed" section of the form. Have the classroom teacher, or another staff member familiar with the student's behavior, complete the form for each of your target behaviors. You may have up to three copies of this form during the FBA process (one for each targeted behavior being evaluated). Once completed follow the directions on the back of the form to help you identify the behavioral function for each of the behaviors being targeted.
Checklist Of Executive Functioning Skills - Complete 1 copy
This checklist helps to identify areas of Executive Functioning Deficit. Classroom teacher or an individual that knows the student well should complete this form and return it to the Teacher of Record. Follow the directions on the last page of the form to help you identify the three most significant areas of executive functioning deficits.
Functional Behavioral Assessment (FBA) Template
Information collection from the above form and data collection is then used to complete this template. This information is then added to IIEP under the Evaluation Tab. This information will also be used to completed multiple section of the BIP located under the Special Considerations Section of IIEP.
