IIEP FBA Evaluation 

Once you and your team have completed the FBA shell document this information needs to be copied and pasted into the evaluation processes section under the Special Education tab in IIEP.  Below is step-by-step instructions on how to do this. 

**Note, we are no longer allowed to simply upload the FBA or BIP shell under the documents section.  It must be entered into the IEP system.  

SITUATION 1 - I AM DOING THE FBA AS PART OF AN EVALUATION - MOST COMMON
In this instance you are completing the FBA as part of the evaluation being conducted along with your school psychologist.  If you are completing an evaluation for the categories of ASD, ED, and/or OHI, you should expect to be conducting an FBA as part of the evaluation process and need to get that information into IIEP.

I HAVE COMPLETED MY FBA, NOW WHAT?

STEP 1
Once you have logged into the students page go the Special Education Tab located on the top banner and find the Evaluation Process tab in the drop down box

STEP 2
This tab will take you to the  evaluations page.  You will need to look at this page closely. The school psychologist may have already added the section for the  FBA.  If not, you will need to add that section.   To add the FBA section you first need to go to the bottom of the page and click the add assessments button. 

STEP 3
Once you click Add Assessment it will take you to the assessment page.  You will need to go to the Custom Assessment Instruments Section at the bottom of the page.  On the left side of the table where it is blank enter, "Direct and Indirect Functional Behavioral Assessment".  On the right side drop down menu highlight Functional Performance/Adaptive Behavior: Functional Behavior Assessment (FBA).

Then click "Save and Continue" This will create a box for your FBA evaluation in the evaluations page.  

Step 4
Add the date you completed the FBA to the date section.  List the completed FBA forms (examples are give in the picture) then click "No Score Type - Text Results" and save.  The button will then change from "Save" to "Details"  When you click on the new details button it will take you the box you will then copy and paste your FBA into.  

Step 5
Copy and Paste the FBA.  The form listed below should be completed by this point.  Simply copy and paste this form in its entirety into IIEP.  Before closing out the field, check that your formatting has remained correct. It should have, but please scroll through and verify.  All sections should be copied and pasted including not only the behavior information but the observation information as well.  

Once all of this information is entered save and continue.  Once a draft is completed or the report is finalized, you will be able to access it under the behavioral considerations page of the IEP.  That will make completing the BIP in IIEP much faster and easier later.  

Once the FBA has been completed and the report generated, you will then use this information to create the Behavior Intervention Plan under the Special Considerations Section within the IEP Process Tab.

HOW TO COMPLETE A STAND ALONE FBA EVALUATION?

Stand alone FBA Evaluations are Evaluations. 
THESE REQUIRE A PARENT SIGNATURE AND ARE ON A 50-DAY TIMELINE. 
You must hold a conference and gain parent permission to conduct your evaluation.  If you are unsure how to generate this paperwork, please use the document below to guide you step-by-step in generating the request for an FBA via IIEP.

Once permission has been received, information has been forwarded to the AWSSC office as with any other evaluation permission the process of completing the FBA is the same as listed in the grade level process pages.  Forms are completed, data is collected, and the FBA shell is completed.  

Once the FBA is completed, the TOR is responsible for creating and generating the report in the evaluation process tab of IIEP instead of the school psychologist.  

How to Generate an FBA only Educational Evaluation Report 

If Pre-Conference Planning was completed and information sent to the AWSSC "Create MD Team", "Assign MD Team Roles" and "Parental Consent Contacts" will already be completed.  Click on Existing Data Button

Exisitng Data Fields
These fields should already be populated by information gathered from the current IEP.  If information needs to be added please due so, but the information in this section should be relativity current since it was updated at the conference requesting the FBA.  If new scores have received in the 50 days (Iready, NWEA, Report Cards, ILEARN, Iread, Etc) or any new progress monitoring information please add them.  

Once information has been added, Save and Continue 

Assessment Data Button
Assessment Data is where most of your work will go.  When opening this section "Evaluation Start Date", Multidisciplinary Team Members", and Referral Information" should already be completed for you. 

To add the FBA Report you will need to click the "Add Domains and Findings Table" button and add the FBA report table

Finalizing the Report
In the "Synthesized Summary Of All Completed Or Reviewed Box" add  - "See summary located in FBA evaluation report."

Once you have review your report and you are sure you are finished, you finalized the report in the system by clicking the "Create Final Education Evaluation Report" button.  This will create your eduacational evaluation report in the system.  

Once the FBA has been completed and the report generated, you will then used this information to created the Behavior Intervention Plan under the Special Considerations Section within the IEP Process Tab.
