This process may only be used with general education students.

The general education teacher should complete the prior intervention forms to assist the team in assessing previous and future interventions.
The teacher data review form can be completed by either one teacher or a team of teachers. It is used to focus discussion and collect ideas about why behaviors may be happening.

This document is created using the information gather from the above documentation and team discussions to come up with a hypothesis as to why the student is engaging in the target behavior listed.
