Our office holds paper copies of these two forms for students to pick up before school, during lunch, or after school.

*All student athletes are require to maintain the minimum CIF GPA requirement of a 2.0 at all times. Failure to meet the GPA requirement puts the athlete at risk of a loss of playing time and potential removal from the team.
*Any student athletes with an open period must see the counseling department or an administrator to verify and sign for their open period.
*Grade check forms are for student athletes to be completed by each teacher. Coaching staff can ask for this of current athletes at any time.
*Parents can also use these forms to check in on their athletes at any time.

Electronic Athlete Release to Parent Form:
*Parent(s)/Guardian(s) can now scan the QR Code to send an email to request to take their student athlete home from any/all away sporting events.
*Emails will only be accepted from the parent/guardian email listed on Aeries.
*By sending this email, it serves as your electronic signature for the paper form below.
*Parent/guardian is still required to show valid photo ID to take student home from the sporting event.
*Students will only be released to the Parent/Guardian. Emergency contacts cannot pick up the student with this form.
Release to Parent Form Additional Information
*This form is only for parents/guardians to take their student home from an away game.
*Student athletes will only be released to the parent/guardian. Emergency contacts cannot use this form.
*This form must be turned in no later than the departure time the day of the sporting event.
*Parent/guardian can email this form signed and completed to Cougar_Athletics@avusd.org no later the departure time on the day of the event.
*Contact the athletics office for more information, if needed.
