Hearing & Vision Health Screening Referrals
Please submit this form AHS Health Screening Referral Form electronically. After I complete the screening, I will communicate the results with you. AUHSD has decided to eliminate the annual 10th Grade Hearing and Vision Screening program. If you feel there is reason to believe your student should be screened, please let Dvora Citron, Acalanes School Nurse know.

I really like this app and they are giving us educators FREE LIFETIME ACCESS. I just applied and it worked RIGHT AWAY. I want to also point you to some excellent resources they have at the bottom of the page of the link above. I really encourage all of you to try this on your own.
