PROJECT FORMS ONLINE SUBMISSION: STEP BY STEP

PLEASE READ THE STEPS CAREFULLY

STEP 0: Download the updated document template (PPF, MOA, and others), fill it out, and save it as PDF. Follow the instructions below to submit this document using the Online Documents Submission.

1. Fill out the Student Activities Online Documents Submission.

2. Read and follow the instructions indicated at the Online Documents Submission form. Incomplete proposals will not be entertained.

3. Attach document/s (in PDF format) you want to submit.

For Project Proposals

- Project Proposal Form with endorsement of Org President and Moderator

- Concept notes, GD mechanics, etc. may be attached if they will help in the expedient processing of the project proposal

For Marketing Packages and/or MOAs

- Make sure that your project has an APPROVED Project Proposal Form before submitting your documents

- Attach Marketing Package and/or MOA without the signature of both parties

4. Project Proposals should be submitted at least one (1) month before the date of implementation to ensure any logistical and financial concerns can be secured at a reasonable time.

5. Marketing Packages and MOAs should only be submitted after you received an APPROVAL of your Project Proposal Form. Submission of these documents should at least be one (1) week before the event to secure any financial and other concerns that may arise.

6. Processing of project-related documents by OSA only takes half a day.

- Documents received between 8:00 AM to 10:00 AM will be processed and released on the afternoon of the same day.

- Documents received after 10:00 AM will be processed and released morning of the next day.

- Please take note that weekends and Holidays are not counted as working day.

After Submission

1. Once you accomplished the Online Documents Submission Form, you will receive an email from OSA acknowledging the receipt of your submission.

2. Expect another email from OSA on the status of your submitted document within the stated processing days.

3. If approved, proceed with the next steps of the activity (Accounting Office for finance). If pending or disapproved, act on the remarks, comments, suggestions, and additional requirements needed as stated on the retrieved document. Consult with your OSA Professional if necessary.

4. In cases when proposals are returned for clarifications and/or require consultation with the organization regarding the project details, the proposal will be approved only after the concerns are addressed. If there are necessary revisions on the form, attach the first draft with the new submission.
