STEP 6: Initiate the ISSC CPT e-form

Once you receive the email with the completed SCAI Graduate Internship document you can initiate the CPT e-Form. 

Visit ISSC's CPT website and follow the instructions posted there.

The  CPT I20 request E-form will ask you to enter advisor information: **You must enter "SCAI Advising" as the advisor name, and enter scai.grad.CPT@asu.edu as the advisor email.**

Please note the following:

1. You must be registered in the appropriate 584 course and your future enrollment must match the number of credits listed on the iPOS before the CPT e-form can be completed.

2. Do not send follow-up emails alerting us that you have submitted the CPT e-Form.

3. Do NOT contact ISSC or the advising office to request they expedite your request. They will process the request as soon as they can. Contacting them to request that your application is expedited will only slow down the process. 

4. Until you have final approval from ISSC, you cannot start working or assume that your authorization for CPT has been extended.
