College Visit Form

Seniors are allowed 2 excused absences to visit a college or university. Juniors are allowed 1 excused absence for this purpose. You must complete this form and return it to the attendance office like you would a doctor's note.

You may also turn in a parent note; however, parent notes are limited, so we suggest using your college visit days first and then using parent notes for any college visits taken after your 1 or 2 official college visit excuses are used.
