Going out of town soon? Fill this out so we can plan ahead! Please keep in mind that many fosters travel for the holidays so the sooner you let us know the better!

Green Alert Form 

If your foster is experaincing a "Green Alert" medical concern please fill this out -- it will go directly to our foster & medical team's inbox as well as can provide you with the next steps needed! 

Examples of Green Alerts: 

• Vomiting for more than 24 hours, more than once

• Cut or puncture wound

• Lethargy/low appetite/not eating

• Coughing/sneezing/nasal discharge

• New limping

• Itchy skin

• Watery/goopy eyes

• Dirty/stinky ears (with or without head shaking)

• Broken nail

• Hot spot or hair loss

• Visible worms in stool

• Straining to urinate/blood in urine

• Diarrhea for more than 24 hours

• Chewed or removed suture staples

Behavioral Concern Form

Have a concern about a behavior of your foster animal? Fill this out and our team will review it and get back to you asap about potential solutions, troubleshooting, or resources that may help. *If this is an urgent behavioral concern that poses any risk please call the shelter!

Examples of Behavioral Concerns: 

• Leash Reactivity 

• Stranger Danger within the home

• Mild resource guarding (e.g., growling when near food or toys but no escalation)

• Occasional house soiling in an otherwise house-trained dog/ house training

• Crate training support 

• Hesitation or fear of new environments or objects after settling into foster home 

• Increased sensitivity to touch or handling

• Jumping or mouthing that is manageable but increasing

Personality Update Form

If you are having challenges or if everything is going great, we want to hear about it! This will help get your foster adopted and help us get to know them better to be a better advocate for them! Feel free to fill this out as often as you like! 

Recommended: 3 days in, 3 weeks in, and additionally if your foster is continuing to come out of their shell!
