Request of Resource Services

• It is the responsibility of the parent(s)/guardian(s) to notify the student's counselor and/or resource teacher at the start of each school year if they are requesting for a student to receive a 504 Plan or ISP.
• Students must have a current evaluation and diagnosis completed by a doctor or licensed/certified professional in the psychological field. This professional could have been one of the parent's choice or may have been the school psychologist. "Current" means the evaluation document is less that three (3) years old. If the document is more than three years old, a new evaluation is required in order to begin services.
• Please make certain that Alleman receives copies of the needed documentation.
• The resource teacher, along with other necessary school personnel, will review the student's evaluation, diagnosis and professional recommendations to determine which accommodations/modifications Alleman can provide and if the student is eligible for a 504 Plan or ISP. '
• please see and complete form under "Resources for Parents" if your child will be starting at Alleman at the beginning of the 2017-2018 school year and will need a 504 or Service Plan.

• Unlike the issue of services, districts are obligated to conduct Child Find in the same manner for families of private school students as it would do with families of public school schools. As a parent or guardian, you have a legal right to request that your public school evaluate your child for special education. An initial evaluation can be completed by a doctor, parent/guardian choice of a licensed/certified professional in the psychological field, or can be requested to performed through Rock Island Public School District (RISD). The student's parents are responsible for the costs of a private evaluation.
• If the initial evaluation is completed by a doctor or private psychologist, The resource teacher, along with other necessary school personnel, will review the student's evaluation, diagnosis and professional recommendations to determine which accommodations/modifications Alleman can provide and if the student is eligible for a 504 Plan or ISP.
• If the initial evaluation is completed by Rock Island Public School District, Alleman, along with RISD, will consult and decide if an evaluation is deemed necessary. If an initial evaluation is deemed necessary, written consent will be needed. The team will meet together for a Domain meeting to go over areas of evaluation and the school district has 60 school days to complete the evaluation and determine eligibility.

If a teacher feels that a student would be eligible for services, the teacher must begin and complete the Alleman R.T.I. process. This process will allow teachers to identify and explain areas of concern as well as show what types of accommodations and/or interventions the teacher has attempted to address the noted concern(s). If the teacher finds that the accommodations and/or interventions used over a period of time will continue to be needed in order for the student to be successful, or feel that the accommodations and/or interventions used did not help the student become more successful, the student and concerns will be further addressed with the resource teacher. If the resource teacher, along with RISD, find that an evaluation is deemed necessary, written consent will be needed.The team will meet together for a Domain meeting to go over areas of evaluation and the school district has 60 school days to complete the evaluation and determine eligibility.
