Arrive at AISJ on Friday Nov 13, 2026 (around 2:45 pm), parents can pick up students or take the bus home

Please complete this CWW Medication Form ONLY IF your child takes medication and will be bringing it on the CWW trip. If you have more than one middle school child, kindly fill out a separate form for each one. This form helps us ensure your child's medical needs are met.
