As part of the Texas Senate Bill 12, or the Parent Bill of Rights, all school organized clubs require a parent permission form to be filled out for each club, and each student for them to participate. If these forms are not filled out your student will be turned away from any meeting or activity until it has been signed by you. You will find all the forms below, if your student is interested in mulitple clubs, a form is required for each club.
