Send this letter to parents so they know we will be working together, and that a form will be sent for them to complete.

Send the Parent, & Teacher Permission Form (454 A) so parents/guardians can sign and return it

Students 18 and older can sign and return the form without a parent’s signature

• After parents have completed and signed the forms, verify the information and sign the forms as the teacher

• Return completed forms to Able SC and allow 10 business days for approval
