Literacy Tracking Tools

Below are assessments and tracking tools (for baseline data and intervention tracking). Each tool is built on a GAFE product so that you can collect data with your mobile device and will be able to easily share that data with the RTI team in the event you need their help. It will force you to make a copy of the tracking tool and, once you do, the tool will be saved in your Google Drive.

Reading

Letter Identification

This tool will allow you to easily track the results of your Incremental Rehearsal Intervention. Be sure you read the intervention instructions on the Literacy Intervention page and follow them exactly.

Phonemic Awareness

Yopp-Singer Test of Phoneme Segmentation will determine the student's ability to hear and segment sounds in 22 single syllable words. Be sure to follow the directions exactly as they are written on the form. This is more of a pre and post assessment than it is a tracking tool.

- Blending Phonemes

This tool will allow you to easily track the results of you Phonics Blending Intervention. Be sure you read the intervention instructions on the Literacy Intervention page and follow them exactly. Don't forget to record the phonetic sound you're addressing.

Sight Words

Fluency

This tool will allow you to track both CWPM and accuracy percentage (both essential to truly understanding a student's fluency). Be sure you are also recording the passage level and its title in the area provided. Ideally, this would be used with a Repeated Reading intervention.

Comprehension

This tool will allow you to track components of reading comprehension through story mapping (linked on the intervention page). You are only recording if the student was able to demonstrate the ability to identify each concept with or without prompting.
