Prior to the first day of class, instructors are expected to print out a paper copy of the Health Status Form for each section they are scheduled to teach. This form must be brought to the first day of class and the information provided on the form must be shared with every student before they can participate in any activity.
Once every student has been informed of the Health Status requirements, instructors must fill out and sign the form. This form, along with the course syllabus, must be uploaded to the Syllabus Uploading Tool. This needs to be done within the first two weeks of the class.
In addition to reviewing the Health Status Form with students and completing and turning in the form, instructors are also expected to include the following statements in their syllabus:
"This course involves a high energy expenditure level and is to be considered high intensity in nature. It is therefore recommended that students know the status of their health and any limitations on their ability to safely participate in the physical activities involved in the course. If there are any questions about your health status, you should seek advice from a physician. You should also inform your instructor of any pre-existing health condition prior to any activity.
You will be expected to participate in (INSTRUCTORS: include the physical activity requirements and intensity/energy expenditure expectations for the course):
You are strongly recommended to consult with your physician or University Health Services prior to participating in the course if you have current or recent injuries or illness and/or ear problems."
