Please print and fill out the Personal History packet. We ask that you arrive at least 15 minutes prior to your appointment, to fill out any additional paperwork.

The purpose of the Client Informed Consent Form (in the attached Personal History packet) is a disclaimer, which states we are NOT A MEDICAL FACILITY/OFFICE. We provide Nutritional/ Holistic services and we will assess and make recommendations for you, through various modalities to help improve your overall health.
