Registration Forms
Please print the attachments listed below. You will need to bring the completed Child Enrollment and Health Information forms, the Registration and Class Preference form, and proof of your child's age (a copy of their Birth Certificate or immunization records). You may contact the Kiddie College administrator at 419-624-1814 if you have any questions. All forms will be reviewed for completeness when they are turned in.
A $50.00 non-refundable fee must accompany the forms to complete the enrollment process.
Please note: Each child must have a Child Medical Statement for Child Care form and their immunization record on file to attend.

Physicals are only valid for one year and to be in compliance with State of Ohio Code 5101: 2-12-58 we are required to keep current medical forms on file in each classroom for all children attending.
