Registration Forms

Forms are available for pickup at St. Stephen UCC or can be printed and completed ahead of time. The required forms are: 

• The Class Preference Form and Registration Information Packet

• The completed Child Enrollment and Health Information Form JFS 01234 

• Proof of your child's age (a copy of their Birth Certificate or immunization records). 

You may contact the Kiddie College administrator at 419-624-1814 if you have any questions.  All forms will be reviewed for completeness when they are turned in. 

A $50.00 non-refundable fee must accompany the forms to complete the enrollment process.    

Please note:  Each child must have an updated immunization record on file to attend. If a family decides they are not doing immunizations, we will need a signed statement on file.
