These documents may be inaccessible to you if have not been inducted by the technician and added to the shared list.

A list of all current Control of Substances Hazardous to Health (CosHH) forms can be found here. Currently we use one CoSHH form for each substance that we use.
New COSHH forms can be made here.
If you cannot find a CosHH form for the substance you wish to use you must complete a new CoSHH form. Once submitted the technician will be notified and will assess the form, print it and give it to you to sign. The form will then be added to the current CoSHH form list. A copy of the form needs to be available in the laboratory where the substance is kept and used.
If you notice any errors in these forms please inform the technician.
