Forms and Instructions

Three forms are required to register for and attend the training: (1) Registration, (2) Logistics, and (3) Health & Emergency Contact. For those who need a visa to enter the U.S. for the training, the visa form is also required.

1. Registration Form - Please submit ASAP; due September 8th

The basic information about your education, software skills, and work experience that you provide in the registration form will be used to structure the training groups.

2. Logistics Form - Please submit ASAP; due September 9th

This will help us arrange your transportation to and from the training venue. Please submit this form ASAP, indicating TBD for all fields you have yet to determine (e.g. arriving flight number, arrival time, departing flight number, etc.). You can then return to this page and re-submit the form as soon as those details have been finalized.

3. Health & Emergency Form (Secure SurveyCTO Form); due September 9th

This information will be extremely important in the event of an accident or medical emergency. Please include any special medical or personal information you would want an emergency care provider to know.

4. Visa Form (Secure SurveyCTO form) - Due ASAP if a visa is needed

If you need a visa to enter the U.S., we will use the information you provide to write a letter to support your visa application.

Note: You are responsible for making sure that you have met the necessary United States visa requirements for citizens of your country.
