- Teacher confers with Special Education, if 504/IEP
- Teacher considers resources found on POS website
- Data Request (send request to the Director of Interventions) Why data?
- Interest Inventory
- Special Education Referral-Part 1 (Background Information)
- "Additional Resources" page of website
- Teacher reflection document (included in student Google folders)
- Teacher consults with student
- Review academic/behavioral expectations
- Include discussion of possible solutions
- Teacher requests access to Google folder, if not already given
- Teacher makes family contact
- Teacher implements classroom interventions
- Teacher establishes a way to monitor progress
- Determine a date when progress will be reviewed and next steps, if necessary, will be taken
- Next step might include: bring to CPT, change classroom intervention, etc.
