- Student with an IEP:
- Case Manager requests a facilitated team meeting by contacting District Administrator
- All other students:
- Lead teacher notifies Director of Interventions
- Lead Administrator conducts interview with nominator and gathers information
- Lead Administrator requests data summary to be updated by data secretary
- Nominator makes family and student contact to inform them of the Intensive meeting format
- Lead Administrator reviews student Google and Cumulative folders
- Lead Administrator sets up a meeting with determined participants(including outside providers)
- Lead Administrator runs a protocol to include creation of an action plan
- Team creates an action plan and identifies a plan coordinator
- Team designee(s) continue folder documentation
- Lead Administrator schedules a follow-up meeting
- Team reviews action plan and implementation of steps
- If no progress is made, interventions may intensify or change or a special education referral may be activated
