- Student with an IEP:
- Teacher requests a special education meeting from the case manager
- All other students:
- Teachers send a request for support to Director of Intervention(Kathy LeMay) via email
- Director of Intervention updates data summary
- Director of Intervention brings student to BLT meeting to identify lead administrator
- Lead administrator talks with teacher to review the teacher reflection and CPT plan, as well as a cumulative file review
- Lead administrator sets up a meeting with key stakeholders
- Students, when appropriate, and families are invited to the meeting
- Lead administrator facilitates an intervention meeting using a protocol
- Notes are taken on the meeting notes sheet
- Team creates an intervention action plan at the meeting and identifies the most logical intervention plan coordinator (classroom teacher or advisor)
- Team continues folder documentation
- Lead administrator collaborates with plan coordinator to ensure full implementation of intervention plan or need for further support
- If no progress is made, new intervention is identified or student is nominated to Intensive Support level or a Special Education referral
