Why have I not received an email?

This depends on who you are and where the document is in the signing process. Please ensure you are checking the email that was provided in the pre-registration process.

What are the common issues you are seeing?

  • Signatures are defaulting to email. Also signatures should not be initials. Please ensure your first and last name are in the signature line.

  • CAPF161 physician and insurance information has been filled out

  • CAPF160 medical condition yes/no boxes are completed

Why were my forms sent back to me?

This answer will vary, but typically there was incorrect information provided or the forms were not complete. We review each form for accuracy and completeness prior to sending the formal email confirmation to parents/guardians and cadets. When the forms are sent back, you must correct all errors noted on the email then resign. Please do not sign prior to making changes as this will lock out your access to make further changes. We will have to resend the forms back to you.

My cadet is not from Ohio Wing- I already paid via PandaDoc but my form was resent. Do I have to pay again?

No. Please complete the form and forward your payment confirmation email to encampment@ohwg.cap.gov. We will make the necessary adjustments.
