(2020-2021 Doctors Form with Instructions) - Athletes will be given a blue card stock version of the physical form from the athletic trainer on campus, but in case it is lost or forgotten you can download this form.

Injured?- Come See Us!
Due to COVID-19 Leander High School Athletic Training will see athletes on an appointment basis to limit our athletic training room capacity. Athletes with injuries that happen on campus will be seen immediately, otherwise all injuries or concerns must be by appointment. To make an appointment please use the Google Form link below. They will be limited to 15 minute time slots in the morning which is mainly for evaluation purposes. High School athletes in need of long term rehab will be scheduled individually during the Athletic Periods as part of our follow up. Before entering for evaluation athletes (and all guardians who are attending with them) will be required to wear a mask, answer COVID-19 screening questions, temperature check (forehead scan) and sanitize hands.
Doctor's Visits:
If your student athlete sees a physician regarding an injury or illness, they must get a medical release form signed by their doctor in order to continue participating in their sport. If unable to download document or athlete/parents do not take medical release form, written documentation must be provided from the treating physician stating any limitations before athlete can return to practice/play.
