Team Funding Request Form

A team must request funds by filling out the Funding Form and submitting it to Ryan Tomczyk, Athletic Director (AD) and the Booster Club Treasurer ((kmbctreasurer@kmsd.edu ). Please review the updated Uniform Guidelines prior to submitting a request for uniforms. Thank you!

Request for Funds are due fourteen (14) calendar days prior to the next Booster Club meeting for evaluation by both the Athletic Director, Ryan Tomczyk and the Booster Club. Please complete the KMHSBC Request for Funds form (link below) and submit it to Ryan Tomczyk, Athletic Director and the Booster Club Treasurer ((kmbctreasurer@kmsd.edu ). 

All requests are reviewed at the regular meeting held the second Wednesday of each month. Note that the requesting team Coach must be in attendance at the meeting, otherwise the request will not be considered. The request will be voted on at the following month's meeting. 

An approved Request for Funds is valid for ninety (90) days. If funds are not used within that time, a new request must be made, following the same procedure. Please note that re-approval is NOT guaranteed.
