Your first appointment - what to expect

Your first appointment will consist of a medical history interview, physical examination and possibly allergy testing and/or pulmonary function testing. At the conclusion of the evaluation, your clinician will review with you your personalized treatment plan and submit any necessary prescriptions to your pharmacy. Please budget 60-90 minutes for your first appointment.

By completing your initial paperwork online using our Patient Portal, you won't have to spend time filling out forms when you should be spending time with the doctor! Online history submission enables us to review your history before your appointment, so we can give you our undivided attention.

Insurance NetwoRk Participation

Kaneland Allergy & Asthma Center participates in many major insurance plans. If you don't see your plan listed below, please contact your insurance company to verify our participation. All insurance referrals are the patient’s responsibility. Our office bills primary insurance; filing of secondary insurance is the patient’s responsibility. If your insurance requires a referral for specialist care, obtaining the referral to Kaneland Allergy and Asthma Center is the patient's responsibility.

Kaneland Allergy & Asthma Center participates in the following insurance networks:

  • Aetna

  • Allied Benefit Systems

  • Blue Cross Blue Shield (PPO/EPO)

  • Cigna

  • Multiplan/PHCS

  • Medicare (existing patients only - currently closed to new patients)

Please bring your insurance card and a photo identification card with you on the day of your appointment. Without these items, our office will be unable to bill your insurance directly.

Our office does not participate with Illinois Medicaid, and state regulations unfortunately do not permit us to see patients with Medicaid as self-pay patients. Search for Illinois Medicaid-enrolled clinicians here.

If you are insured with United Healthcare, UMR, or Humana, please be advised that our office is out-of-network. You can submit a medical reimbursement claim for services received at our office by clicking the following links:

We do offer a discount of 25% off our standard rates for self-pay patients who pay in full at the time of service. Our office will provide a copy of the superbill and receipt for payments made. Completion and submission of claim forms for out of network services is the patient's responsibility.
