• J Forms for Volunteer Drivers

The J Form needs to be completed every year and can be completed and handed in directly to the school.

• Student Activity Release Form (Updated June 4, 2024)

• All parent/volunteers require an up to date Vulnerable Sector Check and Child Abuse Registry Check. Drivers also need to fill out a J Form.

Note:

• The Child Abuse Registry Check and Vulnerable Sector Check are to be completed and submitted to the regional SSRCE Office.

The above noted checks have an expiry of 3 years and need to be done again if they are out of date.
As per SSRSB policy, if these documents are not valid and complete, volunteers will only be able to transport their own child.
**Please be advised that we will not be organizing travel to/from practices or games that take place outside of school hours; this is the responsibility of the families to arrange. It is recommended that students do not transport other students to/from any sporting events. For trips that happen within the school day from 9:30-3:30, a travel plan will have to be in place which will include APPROVED drivers.**
