SAP Referral Form


The Student Assistance Program (SAP) is a process through which a student may seek counsel for themselves or may be referred by a teacher, a qualified school employee, a parent/guardian, or a concerned friend. Students that violate a school policy merit an automatic referral to the team. The heart of the program is the Student Assistance Team, which is composed of school personnel who are specially trained to work with these students.
