What information should I have ready?
You may be asked, but not limited to, the following questions:
1. List of names of every person living in the household.
2. Current Address.
3. Child’s Social Security Number.
4. List of all income for each person in the household.
5. Current and most recent health care insurance for the child.
Are there options for language interpretation?
The Children First - Child HealthWatch Helpline assists families in finding insurance for their children.
