All Alpha Delta Kappa members who have joined the Omega Chapter will be memorialized at the State Convention or Conference. In order to have the information available, each chapter should maintain a file noting personal and professional information for each member. That information should be updated periodically and should include all current members as well as former members if they did not relocate to another chapter.
Upon the death of a member, please notify the State Courtesy Chairperson so an expression of sympathy can be sent. Be sure to include the mailing address where the card should be sent. Submit a copy of Form H-119 to International and to the State President. It is also helpful if a copy is sent to the Necrology Chairperson. Compose a memorial tribute in 100 words or less and email or mail to the State Necrology Chairperson, Jo Wright - at pug4mom@charter.net along with a picture. Guidelines for the tribute include the cause of death, number of years in Alpha Delta Kappa, the name and location of the chapter where the member was initiated. Include offices held and other contributions to Alpha Delta Kappa. Mention should also be made of educational accomplishments and professional experiences. If the deceased had special talents and interests, they should be noted. The names of family survivors can also be included. If a copy of the newspaper obituary is available, please send that also. It will be placed in the State Necrology Scrapbook. All tributes should be received by the State Chairperson at least two weeks prior to the Convention or Conference so it can be included in the ceremony and listed in the Omega Ceremony program. The chapter president or an appointed member of the chapter will read the tribute at the State Ceremony.
