DHS Reporting and Documentation

Report child abuse to the Oregon child abuse hotline by calling 1-855-503-SAFE (7233). The Oregon child abuse hotline receives calls 24 hours a day, 7 days a week, 365 days a week. This toll free number allows you to report abuse of any child or adult to the Oregon Department of Human Services. You can also report child abuse by calling EPD or Lane County Sheriff's Office, Lane County Department of Juvenile Services, or the Oregon State Police.

Step 3: Call DHS 1-855-503-7233

Step 4: Complete District Documentation in Dataport

Follow these directions if this is your first time logging into Dataport:

Click re-set password (this will prompt you to set up your new account)
Once you have your password set up, return to the login screen.
Make sure you enter your username as your name WITHOUT @4j.lane.edu
You will then be able to complete your documentation.

Who can see your report:

You can see the report you submitted
Your school principal
District level support: (only viewed if there is a concern or need for assistance)

Step 5:

Consult with your school admin and call the 4J Title IX Office 541-790-7671, if...

The report is of a sexual nature (touching, fondling, exposure of private body parts, photos of explicit content, dating violence, assault, etc) AND the report only involves 4J Students / Staff / school volunteers.
