EncourAGE,
the Community Health System that tackles social isolation & inactivity
Tackling social isolation & inactivity
For the aging population, loneliness and social isolation, and lack of physical activity, are more than a state of mind; it’s an emerging risk factor that has implications for personal, economic and social well-being.
Tackling social isolation requires connecting people, EncourAGING them to get-out-and-about.
About EncourAGE
EncourAGE is a blueprint and a digital suite to create a healthy aging society with digital services that aim to activate older adults. The activation focuses on both physical activities and social inclusion.
EncourAGE connects older aged persons with like minded, with carers, family and friends.
EncourAGE provides the digital equivalent of traditional human powered social services. It essentially integrates (local) stakeholders; care organizations, local councils/municipalities, cinemas, libraries etc, in the community that aims to promote healthy aging using a person centric approach.
EncourAGE promotes & shares activities that matter to daily lives
EncourAGE brings to the market a new social, motivational, infrastructure, focussing on promoting activities relevant to the everyday lives of older adults. It does this by collecting activities and events and ensures that relevant information is shared with the relevant people (end-users and optionally family/friends).
A digital suite with standard apps and devices
EncourAGE comes with a number of standard apps and devices and can connect 3rd party digital health apps and (monitoring) devices.
Facilitating social inclusion and physical activity
There is great potential to increase physical activity and reduce social isolation by using the dynamics of the elderly population. Invitations (social interventions) can be initiated by community users, or their carers, encouraging them to join an activity. Meaningful activities like "dog walks' ', "gardening", "dancing", and social only activities such as meeting like minded people at the library, visiting a museum or drinks at a nearby pub.
EncourAGE implementation: we'll help
The implementation of a community system is often perceived as a project; this is not correct! It requires a different structure (organization model) and atypical expertise which likely are not readily available.
As an extension to the EncourAGE digital suite we have created a program management outline, using the MSP® methodology. It provides the guidelines for the conception and operation of a Healthy Aging community.
An excellent guideline / framework to use, independent of technology used.
