*You will need to bring to screening: Birth Certificate, Social Security Card, Immunization Records, and Proof of Residency. For the proof of residency, it will need to be a utility bill. If you don't receive the utility bill in your name, then the person you are living with will have to sign a paper to get notarized that you are staying there.
