Health Requirements

District 58 school nurses work with parents and students to keep students healthy and in school.  If your child has a significant medical history, please share this information with the school nurse so that she can coordinate a safe and competent care plan for your child at school. 

Downers Grove Grade School District 58 follows the Illinois school code on the required health examinations.  Please look over the requirements and contact your school nurse if you have any questions.

Health/Physical Examination: The Illinois School Code requires children entering preschool, kindergarten (or first grade if kindergarten was not attended), and sixth grade must have had a physical exam within one year before starting that grade level. Students of any age transferring from a school outside Illinois must also have had a physical within the last year.   Failure to comply with these requirements by the end of the day on October 15th, or 30 days following registration for first-time registrants, of the current school year will result in the student’s exclusion from school until such requirements have been met. Please be sure to complete the parent section on the front side!

Immunizations: All immunizations must be current according to the requirements of the Illinois School Code. Students who fail to comply will be excluded from school if not met by October 15 of the school year.

District 58 Immunization Summary - 2022-23 School Year

Getting an annual flu vaccine is the first and best way to protect yourself and your family from the flu. Flu vaccination can reduce flu illnesses, doctors’ visits, and missed work and school due to flu, as well as prevent flu-related hospitalizations. The more people who get vaccinated, the more people will be protected from the flu, including older people, very young children, pregnant women, and people with certain health conditions who are more vulnerable to serious flu complications. This is not a required vaccination for students but is highly recommended for all.

Dental Examination: Students in kindergarten, second, and sixth grade must present proof of having been examined by a licensed dentist within 18 months of May 15th of that school year (i.e. the 2023-2024 school year would allow for a dental exam to have been performed between November 15, 2022 - May 15, 2024). If a student fails to present proof of a dental exam by May 15th, the school may hold the student’s report card until the student presents proof:  1) of a completed dental examination, or 2) that a dental examination will take place within 60 days after May 15th, 2024.

Eye Examination: All children enrolling in kindergarten or entering the Illinois school system (regardless of age) for the first time must provide the school by October 15, with proof an eye examination completed by an ophthalmologist or an optometrist or proof of an appointment for an eye exam. This does not apply to students entering preschool.  If the student fails to present proof of the required eye examination, the school may withhold the student’s report card until the requirement has been met.
