YES-App Financial Aid Applications

Instructions: YES-App scholars who are requesting financial support from YES-Appalachia for any educational opportunity must complete this form carefully and thoroughly. It is the responsibility of the YES-App scholars to submit this completed form, along with their family's most recent tax return documents, to YES-App staff at least one month prior to the payment due date. All final decisions about financial support will be communicated directly to the scholar and their parent/guardian via email.

Use this same application if you are seeking financial support for college visits. In the "Type of Activity" section, please select "College Visit Support."

It is the responsibility of the YES-App scholar to submit this completed form, along with their most recent FAFSA SAR or CSS Profile, to YES-App staff at least three weeks prior to the payment due date. All final decisions about financial support will be communicated directly to the scholar via email.
