We can provide a PDF copy of your Transcript by email directly to the register of the postsecondary institution, program or scholarship. We can not email transcripts to personal email accounts.
Please contact the Guidance Secretary at heather.cooper@ycdsb.ca if you require an original with school seal.
A fee may apply.
IMPORTANT: EFFECTIVE OCTOBER 4, 2019 -- Colleges or Universities in Ontario no longer accept transcripts directly from St. Theresa of Lisieux CHS
All past students requesting transcripts for the purpose of attending an Ontario College or Ontario University should be using the eTMS system. Through the OCAS and OUAC application process which is initiated by the student, the Electronic Transcript Management System (eTMS) sends secondary school transcript information for past students to Colleges and Universities electronically. Do not complete the St. Theresa of Lisieux CHS Transcript Request form.
If however, you require a transcript for the following purposes:
· Employment
· Post Secondary Education OUTSIDE of ONTARIO
· Scholarship
· Summer School
· Personal / Other
Then please complete the Transcript Request Form to obtain an official transcript.
There shall be no charge for a transcript for current students (2022/23) or for one (1) year following graduation (Class of 2022).
For all other students, there is a $25.00 fee to generate a first copy, and $5.00 for each additional copy. Payment can be made in person in the form of cash or cheque.
Transcripts will need to be picked up at the school. We do not courier them. Please contact the Guidance Secretary to make arrangements if this is not possible.
