Required Forms

Below are links to all forms that need to be completed in order for a student to attend the Fairview Lake Trip in September 2026. Student Permission Form and Medical Disclosure Form must be printed, completed and returned to Mrs. Florio at Eisenhower Middle School by Tuesday, March 31, 2026.

For those filling out the Medication Administration Form, please print, complete, and return it to Mrs. Florio at Eisenhower Middle School by Tuesday, September 8, 2026.

Medication Administration Form

If your child requires daily medication (prescribed or over-the-counter) or emergency medication (Inhaler, Epi-pen, etc.) please complete the Medication Administration Form. This form does not take the place of the Medical Disclosure Form which is required for all students. ONLY complete the Medication Administration Form if your child will require medication (daily or emergency) on the Fairview Lake Trip.

*Forms and payment can also be returned to the main office of your child's elementary school. A box has been placed in the office for collection and will be checked frequently for forms and funds.
