COVID-19 Public Health Recommendations for Local Health Departments for K-12 Schools.
In accordance with the NJ Department of Education requirements for reopening our schools, and as outlined in the district plan, we must implement a procedure for safely and respectfully screening students on a daily basis for COVID-19 symptoms. The first step in our protocol requires that parents / guardians use the district-provided screening checklist to conduct a daily health screening each morning prior to sending their child to school for in-person instruction. Staff will be on-site when students enter the building and each classroom to conduct visual screenings of all students. Any student who presents COVID-like symptoms will also be screened by the school nurse.
By clicking this form, I understand that it is my responsibility to conduct a daily home screening of my child before school in accordance with the district provided screening checklist. Furthermore, if the screening indicates that I keep my child home, I agree to do so and notify the school immediately.
